Within 60 days of the date of your denial, send the 561 to your local social security office. The address should be on the notice you received. Send it certified mail return receipt to ensure proof of filing.

If mailing it, please be sure to mail it certified return receipt requested. You will want PROOF that you sent them the appeal papers. You can also do it online at ssa.gov (click 'benefits', 'appeal...').

I also suggest you use an attorney, since handling it yourself doesn't seem to be working. Many of us handle Hackensack cases, so just pick up the phone.

As the other attorneys have advised, you should return the form to your local social security office. If you do not know where the office is located, I suggest you look it up on the government's website: www.ssa.gov. You can also call the toll free number (1-800-772-1213) and speak with a representative. Given the wait time for most phone calls, however, you are better off finding the information online.
